AnnotationThis report covers both hospital and community services in Ayrshire and Arran, with many examples of good practice in acute assessment and rehabilitation services. Problems are noted on patient transfers within and between hospitals, delayed discharge, increased numbers of acute admissions, and the quality of life for many patients. The absence of a joint strategy for older people is being tackled, and builds on existing joint planning and good joint projects. Ayrshire and Arran Health Board have the following three priorities: to redesign health and social services which meet the needs of older people; to develop integrated care pathways across all sectors; and to prevent inappropriate hospital admissions by enhancing partnership working across sectors.
