AnnotationA main concern in the home health care industry is relapse prevention. This article provides information concerning cognitively intact older people's psychosocial needs that could contribute to relapse episodes. All participant disciplines in the treatment team play a vital role in relapse prevention, beginning with discharge planning. However, the role of the social worker, providing social and emotional interventions to speed recovery and/or prevent relapse, has become more prominent. (RH).
