Annotation | Anglesey became a unitary authority in April 1996, before which Gwynedd County Council was responsible for social services provision. Joint reviews provide an independent assessment of how well the public is being served by social services locally. This review found that many people in Anglesey are well served, but with some concerns about how well some people are able to access services that meet their needs. Most service users and carers are very satisfied with the services they receive, but were unaware of the existence of the out-of-hours service. Examples of good practice include: Model Mon, a community-based multidisciplinary assessment; "fast track" assessments and equipment for bathing; and support provided by Carer's Outreach in training carers how to lift safely. The review notes the need to develop a range of services to support an increasingly ageing population within available resources, at the same time as promoting choice and independence. Key issues identified as priorities include: further attention to care management; and strengthening the commissioning role and establishing commissioning priorities.
