Annotation | The Department of Health (DoH) consultative document was commissioned by Frank Dobson (previous Secretary of State) following the 1997/98 winter bed crisis. Of the three options suggested for care of older people, the British Geriatrics Society (BGS) supports the third: care closer to home. If delivered effectively and consistently, this would seem to be the option that future older people are most likely to welcome. The BGS is generally supportive of the document's guiding principles, but is critical of the analysis, which offers little understanding of the factors which have led to past problems, for example, the number of inappropriate hospital admissions. The BGS propose that the objectives of reduced disability and greater independence are the key clinical outcomes against which future service arrangements should be judged. Consequently, it recommends provision of adequate post-acute rehabilitation; and making better use of the dearth of specialist personnel. Further research and evaluation is key to ensuring effective development of the service. (RH). |
