Annotation | The starting point for this document is the importance of enabling older people to continue living independently in their own homes, but recognising that appropriate residential care is required for those who cannot. The aim is to develop an integrated health service for over 65s in Camberwell, by achieving an effective partnership between community-based and age-related hospital-based services.
