AnnotationSocial work skills - individual and family focused - are demonstrated. The author describes how, as a professional social worker, she has drawn on traditional social work values, knowledge and skills to create a new model of home health care for sick and frail older people in New York. The establishment of Concerned Home Managers for the Elderly (COHME) as a non-profit, licensed home health care agency, was based on her observations of a home health care system in Bologna, Italy. Of primary concern to COHME are quality of training, ongoing education, and daily work life of COHME aides, as well as how long-term care is provided for clients. The book explores areas of case management: there is an emphasis on a high level of interaction between all staff to share their expertise. (RH).
