Annotation | Outside hospitals, there is great variability in assessment tools used by social service departments (SSDs) and the documentation in nursing homes. In the US, a uniform, comprehensive, standardised assessment for routine long term care of older people, the Minimum Data Set - Resident Assessment Instrument (MDS-RAI) has been introduced into all nursing homes. While improvements in care have been reported, there are also unanswered questions concerning use of some treatments. In the UK, the trend is towards use of performance indicators and evidence-based medicine. Standardised assessment scales using the same set of items as the MDS-RAI have now been developed for community care, mental health, acute and post-acute care of older people; and systematic evaluations are now under way.
