Annotation | This article describes the development and conceptual framework for a community initiative to improve care of older people in largely rural areas. A process of exploration and interaction with local communities led to a system development "Models Project" in which a regional geriatric programme worked with communities to develop local geriatric resources. Organisation development principles were used to guide the project. An expanded view of comprehensive geriatric assessment provided a vision for community development of services for older people. The "Models Project" serves to illustrate principles and processes that may be of value for other communities with scarce specialised geriatric resources that are seeking to develop geriatric assessment services and improve care for older people.
