Annotation | Little information exists about residents with diabetes in long-term care settings. However, several recent studies have provided evidence of unstructured diabetes care, lack of clear boundaries of clinical responsibility, and of residents enduring a tremendous personal health burden as a result of diabetes. This report of the Working Party chaired by Alan Sinclair summarises and reviews the evidence base of published studies and documents. The main findings are that residents with diabetes in institutional settings appear to be a vulnerable and neglected group, characterised by prevalence of cardiovascular complications, susceptibility to infection, increased hospitalisation rates, and high levels of physical and cognitive disability. Key recommendations include: use of an individualised diabetes care plan for each resident; development of a protocol of diabetes care within each care home; establishing a policy of screening for diabetes; at least one diabetes specialist nurse (DSN); and a training programme. The recommendations should be seen as framework for developing national standards of diabetes care within care homes in the UK. (RH). |
