Effects of functional status changes before and during hospitalization on nursing home admission of older adults

AnnotationFunctional status changes before and during hospitalisation may have important effects of outcomes in older people, but these effects are not well understood. The authors determined the influence of functional status changes on the risk of nursing home (NH) admission after hospitalisation in 551 general medical patients aged 70+ admitted to hospital from home. Functional status change categories were: stable in function before and during hospitalisation (45% of study patients); decline in function before and improvement during hospitalisation (26%); stable before and decline during hospitalisation (15%); decline before and no improvement during hospitalisation (13%). In multivariate analysis, those in the decline /no improvement group, and the stable/decline group were at greater risk for NH admission than those in the stable/stable group. In a multivariate model that controlled for ADL function at hospital discharge, functional status change was no longer statistically significantly associated with NH admission. Monitoring the functional status of older patients could optimise use of hospital and post-hospital care to maximise continued independence living at home. (RH).
