Annotation | There is strong evidence that comprehensive assessment, when followed by implementation of individual care plans, reduces the risk of older people being re-admitted to hospital or admitted to care homes. However, the process of carrying out such assessments continues to present challenges to those involved, who may frequently be required to work in different settings and across professional and agency boundaries. This article explores the situation in the light of current working practice and policy directives. A set of benchmarks for assessment practice has been suggested, and included in the article. More work will be needed if small-scale examples of good practice are to be applied across whole systems. (RH). |
